Prurigo pigmentosa is an inflammatory skin disorder that presents with an eruption of erythematous papules and plaques that resolve into persistent reticulated hyperpigmentation. Due to its rarity and the clinical and histopathological overlap with other skin conditions, misdiagnosis of prurigo pigmentosa is common. Many cases of prurigo pigmentosa are linked to underlying metabolic derangements, including ketosis secondary to diabetes mellitus and ketogenic diets; normalization of the ketotic derangement facilitates the resolution of the dermatosis. Evidence-based guidelines for treating prurigo pigmentosa are lacking, and therapeutic regimens must be personalized. This activity reviews the etiology, pathogenesis, evaluation, histopathologic correlates, and management strategies for prurigo pigmentosa and its resulting hyperpigmentation. The activity also highlights the role of the interprofessional healthcare team in caring for and improving outcomes for patients with prurigo pigmentosa.
