Psychiatric Patients: Boarding in the Emergency Department

Boarding of psychiatric patients continues to be a widespread problem in emergency departments across the country. The definitions of boarding all share the common feature that a patient with a mental health issue is held in the ED while waiting for additional evaluation, admission or transfer. Many factors contribute to boarding; the impact is negative not only on the patient but on all stakeholders. Since the challenges of boarding are unlikely to disappear anytime soon, all ED personnel, including physicians, advanced practice clinicians and nurses, will benefit from more education on the evaluation and treatment of the psychiatric patient in the ED.

In this activity, we encourage your use of critical thinking skills involving evaluation and treatment of the psychiatric patient in the ED. We will cover the reasons for boarding and its impact on everyone involved. We will look at psychiatric admissions and examine treatment of the psychiatric patient in the ED. After completing this activity, practitioners should be able to: appraise the scope of the problem of psychiatric patients boarding in the emergency department; summarize the admission criteria for psychiatric patients; and formulate alternative treatment options for psychiatric boarders.
