Multiple myeloma is a plasma cell disorder characterized by the clonal proliferation of malignant plasma cells, producing monoclonal proteins and causing organ damage. In addition, the more recently categorized “monoclonal gammopathy of renal significance” has its own associated diagnostic and treatment challenges. The involvement of the kidney in plasma cell dyscrasias, including multiple myeloma, is widespread. Cast nephropathy is the most common renal involvement; however, many other immune-mediated and non-immune–mediated disease patterns exist. Given the significant overlap of clinical presentations, histologic and serologic evaluations are crucial in identifying the underlying pathology. Treatment options have greatly expanded in the last decade, and ongoing research may lead to further developments. This activity examines when myeloma kidney should be considered, how it should be managed, and the role of the interprofessional team in caring for patients with this condition to ensure the best outcomes.
