VHA is charged with providing care for Veterans. Geriatric Clinical Pharmacists and Clinical Pharmacy Specialists, including those working in Home-Based Primary Care, Community Living Center, palliative care, etc., support key VA initiatives. This clinical series will provide new and updated clinical information and current practices for the Geriatric Pharmacist caring for the aging Veteran population. The purpose of this live-virtual knowledge-based webinar is to address staff needs by providing didactic and Veteran case series discussions to build on baseline Geriatric Clinical Pharmacist Practitioner (CPP) knowledge to promote Comprehensive Medication Management (CMM). This series is needed because it specializes in Veteran care needs for Geriatric CPPs to use their global scope of practice in the patient care process to improve access to high quality Geriatric CPP services use of CMM in the clinical care process. Identifying the need for this training comes from FY24 data from the PharmD tool use: Over 1,051,000 Pharmacist interventions occurred in the VA last year including disease states prevalent in the geriatric population, including diabetes, hypertension, mental health, pain management, and both cardiovascular and respiratory issues. Unfortunately, currently many Geriatrics CPPs have focused disease state visits and they consider only one disease state. The expected level of Veteran care during CMM is to address more than one disease state and improve access for Veteran care (see below). The changes anticipated in the learner include disease state expansion. We also anticipate improved encounter productivity, and increased number of Veterans served through improvement of Geriatric CPP knowledge and efficiency during the patient care encounters
