S30 Use EHRs to Prevent Diabetes, Reduce Costs, and Advance Health Equity

The CDC’s PreventT2 lifestyle change program provides high-quality, evidence-based care to prevent or delay T2D for those at risk. However, it can be challenging for hospitals and health centers to identify, engage, and retain eligible participants for this yearlong program. By adopting a population health approach, stratifying risk, and collaborating with clinical and community partners, ADCES, Trinity Health, and Comagine Health, in partnership with Providence Medical Group, utilized Electronic Health Records (EHR) systems to improve access to quality care while reducing costs and advancing health equity. In this session, attendees will learn how they can put technology to work to integrate community health, preventive care, and primary care for optimal patient outcomes.
