For this initiative, we broadly define “medical home” as where a child regularly receives care. Patient-centered medical home designation (PCMH) is not required. Healthcare providers and staff from practice-based medical homes, such as primary care facilities (both private and non-private), specialty clinics, community hospitals, Federally Qualified Healthcare Centers (FQHCs), and rural health providers within Illinois, are eligible to apply. While the practice/provider is not required to exclusively cater to pediatric patients, applicants must provide care for them.
