Strengthening Pediatric Medical Home Disaster Preparedness: Illinois Pilot

Join us in strengthening pediatric disaster preparedness in practice-based pediatric medical homes. By acknowledging the crucial role of the medical home and ensuring that necessary resources are available, we can enhance the level of care for children during disasters and aid in community recovery. Take part in webinars and interactive online learning sessions to help your practice increase your readiness! For this initiative, we broadly define “medical home” as where a child regularly receives care. Patient-centered medical home designation (PCMH) is not required. Healthcare providers and staff from practice-based medical homes, such as primary care facilities (both private and non-private), specialty clinics, community hospitals, Federally Qualified Healthcare Centers (FQHCs), and rural health providers within Illinois, are eligible to apply. While the practice/provider is not required to exclusively cater to pediatric patients, applicants must provide care for them.
