Strokes, new or recurrent, affect about 700,000 Americans every year. Stroke is the third leading cause of death in the United States and the number one cause of adult disability. With more than a million hospitalizations occurring in the U.S. each year, the importance of stroke as a major cause of morbidity and mortality cannot be overemphasized. Medical errors in this high-risk area may lead to profound disability or death. In addition, lawsuits in this arena are exceptionally costly. Practitioners should take the same risk-managed approach to every patient with a neurologic presentation or signs and symptoms of stroke or transient ischemic attack (TIA).
This activity is the first of our two-part program on stroke. In this activity, Stroke Part 1, we explore the most common reasons the diagnosis of stroke is missed, delayed, or inappropriately managed, as well as the common medical errors and allegations of malpractice; we also provide risk and documentation guidelines. We present and discuss cases related to the atypical stroke/TIA presentation, which is difficult and sometimes impossible to diagnose, and the failure to diagnose stroke. After completing this activity, practitioners should be able to: apply knowledge of the common reasons for medical errors and litigation related to stroke and TIA to the evaluation and management of patients with a neurologic presentation or signs and symptoms of stroke or TIA in order to improve patient safety, improve documentation and decrease the risk of exposure to medical malpractice; identify the signs and symptoms of posterior circulation stroke and TIA; and incorporate a detailed neurologic examination, including a stroke scale, into the medical records of patients with a neurologic presentation or signs and symptoms of stroke or TIA.
