Stroke Care: Telestroke

Stroke is a significant cause of morbidity and mortality in the United States. Each year about 795,000 people experience a new or recurrent stroke. In 2010 there were 671,000 emergency department (ED) visits with the diagnosis of stroke, which puts ED personnel on the front line in the recognition, treatment and outcome of a stroke patient. The availability of personnel and resources for acute stroke diagnosis and emergency management varies by institution across the U.S. Stroke centers (primary and comprehensive) that are adequately equipped and staffed represent only a minority of all hospitals. Stroke expertise may not be available in non-stroke centers that are located in some urban and in most rural communities. Just over half (55.4%) of the U.S. population has access to a primary stroke center within 60 minutes from symptom onset. Telestroke is a tool that can help address this issue.

In this activity, we inform practitioners about recommended telestroke uses for the treatment of acute stroke. We include a comprehensive discussion of telestroke basics, teleradiology, assessment, and thrombolytic administration for the stroke patient. After completing this activity, practitioners/members of the healthcare team should be able to: identify best practices to be considered for assessment and management of acute stroke when planning for the use of telestroke consultations; discuss the evidence that supports the reliability of telemedicine for the diagnosis and efficacy in acute stroke treatment; and summarize the metrics to use when determining performance, outcomes and quality of telestroke.
