Stroke Literature Review: Acute Stroke Management with Thrombolysis

Strokes, whether new or recurrent, affect about 700,000 Americans every year. Stroke is the third leading cause of death in the United States and the number one cause of adult disability. With more than a million hospitalizations occurring in the U.S. each year, the importance of stroke as a major cause of morbidity and mortality cannot be overemphasized. Developments in the area of thrombolytic administration in early stroke management have led to much controversy about lytic therapy and a striking difference in stroke management in hospitals across the country. The literature has made it clear that the medical community should be providing better care for stroke victims across the full spectrum of stroke management. It is critical for practitioners to stay current with the literature in these areas and to know hospital policy and protocols related to the use and administration of lytic agents for these patients.

In this activity, we explore the research, guidelines, and recommendations behind the early controversies regarding use of lytic therapy as well as the current literature and most recent recommendations for lytic administration in stroke; we begin with the early literature, then the controversy, and finally the ultimate acceptance of the therapy. We then summarize the state of the art regarding stroke management. We also discuss key medical-legal issues and offer recommendations to improve patient safety and reduce the risk of litigation. After completing this activity, practitioners should be able to: create or modify an existing stroke management protocol based on the current medical literature regarding the use of thrombolytic agents in patients presenting with stroke in order to provide timely and appropriate stroke management; design a system solution for the management of stroke patients that allows a hospital organization to deliver high-quality stroke care; and classify the potential medical errors and reasons for malpractice litigation related to stroke treatment, in order to minimize the medical-legal risks associated with the use of thrombolytic agents.
