The period before the patient enters the pre-operative phase is a crucial time for the ultimate success of the surgery. The patient s condition and suitability for surgery needs to be carefully scrutinized. All alternative therapies need to be considered and, if appropriate, offered to the patient. The potential risks of the surgery need to be weighed against the potential benefits in an objective fashion and explained to the patient. It is only after due (and careful) consideration of all of these issues has been completed that the patient should be scheduled for surgery. In this activity, we review the considerations that are necessary in surgical decision-making. In addition, we discuss the following topics: assigning priority for an appointment; pre-operative clearance; discussing alternatives; contraindications; and terminating the patient relationship. After completing this activity, practitioners should be able to: evaluate and modify the office practice to ensure that patients receive care in a safe and timely manner; appraise all available medical information about a patient before finalizing the decision to perform surgery; assess non-operative alternatives before proceeding with surgery; and categorize the acceptable reasons for terminating the patient relationship.
