Surgery: Risks in the Intraoperative Phase

The intraoperative phase is the period from when the patient enters the operating room until the patient is transported to a postoperative care unit; it plays a major role in the success of the procedure. It is a crucial part of the surgical process and is probably more heavily dependent on teamwork than any other aspect of surgery. Risk issues can occur after the surgical procedure is completed but before the patient has been released from care for the procedure, including errors that arise when communication patterns are defective.

In this activity, we review cases that deal with the systems and processes by which surgery is performed with an eye to making it safer for the patient and less litigious for the practitioner. We review the varied risks involved, including: retained foreign bodies; fire; intraoperative monitoring; intraoperative biopsies; inadvertent organ damage; and hemorrhage. After completing this activity, practitioners should be able to: the participant will be able to explain the need for disclosure to the patient of persons performing major aspects of the surgery; the learner will be able to explain the ways in which hand-off communications should be managed; the learner will appreciate the three elements needed for a fire and the ways to safely manage them; the learner will appreciate the need to perform surgery in the manner discussed with the patient, to the maximum extent possible; and the learner will be able to explain the ways in which retained foreign bodies can be prevented.
