Chronic venous obstruction (CVO) is a significant cause of chronic venous insufficiency (CVI). CVO shares presenting symptoms with CVI; however, leg edema is more prominent and debilitating in CVO. Historically, CVO and CVI were managed with conservative therapy centered on lifestyle changes and compression therapy. Surgical intervention was limited to symptomatic patients progressively worsening with conservative management. In recent years, technological advancements in endovascular interventions have bridged the gap between conservative and invasive surgical management of CVO. However, patients who do not respond well to conservative and endovascular management may still require invasive surgical intervention. In addition, novel surgical techniques have recently been described for treating innate and iatrogenic valvular disease in patients with CVI. This activity reviews the pathophysiology, preoperative evaluation, and surgical interventions for symptomatic CVO refractory to conservative and endovascular treatment and highlights the role of the interprofessional team in caring for patients with CVI and CVO.
