Testicular Torsion: Diagnostic Error and Risk Reduction

The diagnosis of testicular torsion can be extremely difficult to make. Misdiagnosis of testicular torsion in male adults has been reported to occur in 15% to 75% of cases. The failure to diagnose torsion continues to be a significant source of litigation against emergency and primary care physicians. The risk of testicular loss is extremely high if surgical intervention does not occur within hours of symptom onset; therefore, it is critical for practitioners to make a rapid, accurate diagnosis and arrange for appropriate consultation.

In this activity, we focus on the misdiagnosis of torsion as epididymitis, the timing of testicular salvage, risk factors, the clinical presentation, the physical examination, the laboratory and diagnostic evaluation, and treatment. We present case examples and risk management recommendations related to the evaluation, diagnosis, and management of testicular torsion. After completing this activity, practitioners should be able to: evaluate male patients, keeping in mind that testicular torsion can present as abdominal pain rather than scrotal pain in order to reduce the incidence of delayed or missed diagnoses of testicular torsion; compare and contrast the features of epididymitis with those of testicular torsion in order to distinguish between the two conditions in the evaluation of male patients who present with abdominal or scrotal pain; and employ risk factor analysis for testicular torsion as a part of history-taking for male patients who present with abdominal or scrotal pain.
