Recent guidelines from GOLD have recommended using serum eosinophil measurements to drive therapeutic decision-making. There is a paucity of clinical research that supports routine use of eosinophil measurement in patients with COPD, and it is still unclear if there is a distinct phenotype of eosinophilic COPD and whether serum eosinophils are an ideal or acceptable marker to identify such a phenotype. The optimal approach to therapy in these individuals is a subject of ongoing debate, including use of inhaled and or oral steroids as well as potential use of biologics.
