Thin Basement Membrane Nephropathy

Thin basement membrane nephropathy (TBMN), also known as thin basement membrane disease, is a genetic disorder that affects the glomerular basement membrane (GBM). TBMN is characterized by microscopic hematuria, typically with normal kidney function and the absence of proteinuria. TBMN results from mutations in genes encoding type IV collagen, leading to widespread and uniform thinning of the GBM. TBMN usually presents as isolated microscopic hematuria, which may be dysmorphic, without significant proteinuria, renal dysfunction, or hypertension. Although TBMN shares some overlapping features with Alport syndrome, such as its genetic basis and involvement of type IV collagen, the course and prognosis of these 2 diseases differ significantly, requiring distinct management strategies. Unlike Alport syndrome, TBMN generally does not involve extrarenal symptoms and follows a more benign clinical course.The diagnosis of TBMN is often based on clinical presentation and family history, with renal biopsy showing diffuse thinning of the GBM. A family history of progressive renal disease is usually absent, and in such cases, a renal biopsy may not be necessary for a definitive diagnosis. Management is typically conservative, as most patients do not require specific treatment due to the benign nature of the disease. This activity reviews the pathogenesis, evaluation, and management of TBMN and also highlights the critical roles of the interprofessional healthcare team in evaluating and treating patients with this condition.
