Thoracic aortic dissection (TAD) is the number one cause of death in individuals with Marfan syndrome. The failure to diagnose TAD continues to represent a significant problem in the practice of medicine. This remains a critical issue from both patient safety and medical error perspectives. The main purpose of this activity is to increase awareness of the Marfan syndrome as a major risk factor for TAD. Practitioners must take the same risk-managed approach to every patient with Marfan syndrome presenting with pain in the chest, back, and/or abdomen.
In this activity, we present and discuss two cases of patients with Marfan syndrome and TAD; in both cases, the diagnosis of TAD was delayed or missed. In the case discussions, we explore clinical decision-making and highlight the role of cognitive errors. We also provide evidence-based/best practice recommendations. After completing this activity, practitioners should be able to: compare and contrast the characteristics of patients with and without Marfan syndrome who present with acute aortic dissection; appraise the signs and symptoms consistent with malperfusion syndromes found in patients with acute aortic dissection; and analyze when a patient encounter may involve heuristics and biases, or cognitive dispositions to respond, in order to improve clinical decision-making and reduce the incidence of the failure to diagnose thoracic aortic dissection in patients with Marfan syndrome.
