TOLAC is defined as a trial of labor after one or more cesarean deliveries intended to result in a vaginal birth instead of a scheduled repeat Cesarean Section. Successful vaginal birth after Cesarean (VBAC) rates in TOLAC patients are reported to be 60% to 80%. The safety of TOLAC/VBAC is supported by very low uterine rupture rates of 0.5-0.9% in patients with one previous low transverse uterine incision and 0.9%-3.7% in patients with more than one cesarean delivery. Despite the safety and success rates in appropriate candidates, the rate of VBAC remains very low. Promotion of safe TOLAC is a key intervention in the efforts to reduce cesarean delivery rates and the associated increase in maternal morbidity and risk of litigation.
