Most recent estimates show that over 25% of U.S. adults have multiple chronic health conditions and approximately 50% of adults suffered from at least one of several highlighted chronic conditions. These conditions remain a public health concern and lead to increased healthcare utilization and spending. Some of the primary goals of the national initiative focused on addressing MCC (Multiple Chronic Conditions) in the United States include strengthening health care and public health systems to improve gaps in effective treatment and management on a point of care as well as systems-based level. Constantly updated guidelines on treatment and management of these conditions necessitate primary care providers to continuously update their education from different knowledge sources. Interaction with human experts specifically for complex medical questions has been cited by physicians as a more effective mode of knowledge transfer. This activity will support one of the goals of the Department of Health and Human Services in providing primary care providers better tools and resources to improve care of individuals with chronic medical conditions.
