Uremic Pruritus Evaluation and Treatment

Uremic Pruritus Evaluation and Treatment

Uremic pruritus, also known as chronic kidney disease (CKD)-associated pruritus (CKD-aP), is a common and debilitating symptom experienced by patients with CKD and end-stage renal disease (ESRD). Uremic pruritus is characterized by itching that is not attributable to other causes and can be either localized or generalized. Uremic pruritus significantly impacts patients’ quality of life, affecting sleep and leading to poor clinical outcomes and adherence to treatments. Additionally, uremic pruritus is associated with poor adherence to treatments and worse clinical outcomes. Therefore, it is essential to recognize and treat uremic pruritus promptly.Several proposed pathogenetic mechanisms may explain uremic pruritus, and these mechanisms are potential targets for therapy. Before diagnosing CKD-aP, it is crucial to exclude other causes of pruritus, such as eczema, liver and thyroid diseases, and atopy. Management of uremic pruritus involves a stepwise approach, starting with topical emollients, followed by medications like gabapentin and phototherapy. In advanced cases, patients may undergo transplantation, as a successful renal transplant can relieve them from CKD-aP. This activity reviews the etiology, evaluation, and management of uremic pruritus and emphasizes the importance of collaboration within an interprofessional healthcare team to treat patients with this condition, thereby ensuring comprehensive care and improving patients’ quality of life.
