Von Willebrand factor (vWF) mediates platelet adhesion to exposed subendothelial collagen at sites of vascular injury, facilitating the formation of primary hemostatic plugs. Von Willebrand disease (vWD), a bleeding disorder, arises from quantitative or qualitative defects in vWF. Replacement therapy with vWF is used to manage and treat vWD, especially for individuals refractory to desmopressin. vWF is available in plasma-derived concentrates and, more recently, in recombinant form.This activity emphasizes the indications, contraindications, dosing, and monitoring associated with vWF replacement while helping clinicians understand the intricate pharmacology of vWF to tailor and administer treatment plans to individual patient needs.
